I've done about 200 threads so far, and the ratcheting of this needle driver is still intact as it was the first day I bought it. Phenomenal quality in comparison to the ones that come with a suture pad kit. Very similar to the OR, if the OR is a 10/10, this is an 8/10. You won't regret it for the price. Excellent build quality
